CAMP KAIZEN – GYM WAIVER & LIABILITY RELEASE

By using the Camp Kaizen gym facilities, you agree to the following:

ASSUMPTION OF RISK

I understand that participation in physical exercise and use of gym equipment involves inherent risks, including but not limited to injury, illness, or in rare cases, serious harm.

I confirm that I am physically fit and able to participate in exercise activities and have no medical condition that would prevent safe use of the facilities.

USE OF FACILITIES

I agree to:

• Use all equipment responsibly and as intended

• Follow any instructions provided by staff

• Respect the space, equipment, and other users

• Stop exercising immediately if I feel unwell or at risk

LIABILITY WAIVER

I acknowledge that I use the Camp Kaizen facilities entirely at my own risk.

To the fullest extent permitted by law, I waive any and all claims against Camp Kaizen, its owners, staff, or affiliates for any injury, loss, or damage arising from my use of the facilities.

PERSONAL RESPONSIBILITY

I accept full responsibility for my actions and wellbeing while using the gym.
